What information should be included in my charge?

You should include as much relevant data as possible. Be sure to include information about how to contact you, the name and address of the discriminating party, the type of discrimination, relevant dates and witnesses, and specific facts. If pertinent, you also might include employment contracts, brochures, or similar documents that demonstrate company policy. Before you file the charge, make sure you sign it.
