If I am filing for disability benefits, what other documents should I bring with me?

In addition to the documents listed above, you should try to bring a list, with addresses and telephone numbers, of the doctors, hospitals, or institutions that have treated you for your disability; a summary of all the jobs you have held for the past fifteen years and the type of work you performed; and information about any other checks or payments you receive for your disability. The next section of this chapter provides more information about disability benefits.
